This page is designed to help our builders and our commercial services customers to readily access the application forms needed to begin the process of obtaining the taps that they need for construction as well as other helpful forms needed for compliance and inspections.
Builder Services
For the application form for water taps for residential properties, a plumber’s Certification of Compliance, a request for a Sewer Inspection or a Customer Service Inspection, please click on the link below to access the application form. Please complete the appropriate form(s) and return with all necessary information and fees to the address shown on the form. Please remember that a plumber’s Certificate of Compliance form is required for each tap. A blank copy of that form is also available below.
Please remember that all taps, meters and vaults are to be installed by Hays Utility South.

Please review the Customer Service Inspection Scheduling Days form below to find the days we normally schedule CSI’s in your District. If you have a strong need for a CSI on a different day, please call us and we will work with you the best we can.
Commercial Services
For the application for taps for a commercial structure, please click on the link below to access the application form. Once completed, please return with any required information and we will calculate the required construction deposits (if any) and all applicable tap fees required. Please send this to the address on the cover page.
